Referral

Please consider the following prior to making a referral:

  • Have you connected with their parent/guardians to discuss concerning behavior?
  • Have you tried various, coordinated interventions, in school, to help the child succeed?
  • Have you connected them with a local mental health provider? 

If you can answer YES to all of these questions, please proceed with making a referral:

  • Referrals are being accepted from families and providers for children and youth who...
  • Are Medicaid eligible
  • Age 6 to 21 years of age and served by the Claremont schools in some capacity
  • At-risk or experiencing a serious emotional disturbance
  • Developmentally appropriate to participate
  • NOT being served long-term by another agency (i.e. Out of District placement)

Although FAST Forward staff can serve only a small cohort of youth in its first year of implementation, they will provide facilitated referrals and interim supports to youth waiting for FAST Forward services.

FAST Forward staff and partners have been trained in the nationally recognized and evidence-supported wraparound process and will be working to build up the network of wraparound partners.
